First Visit

Your First Visit at Azure Dental Studio in North Waltham, MA

When you come in for your initial visit, please be sure to arrive early to fill out your initial paperwork, if you have not already done so, and bring your insurance card. If you have a previous dentist, please ask your former dentist to send a copy of your records.
